Eylem Emel Ar\u0131kan(Ceren)<\/strong> (\u2018Data Controller\u2019), who has the title of data controller within the scope of the Law No. 6698 on the Protection of Personal Data (\u2018Law\u2019) and the international conventions regarding human rights to which our country is a party, and the information on the rights and obligations of both you and the Data Controller regarding this data processing activity is given below.<\/p>\n

I. Methods of Collection of Your Personal Data and Their Legal Reasons<\/strong><\/p>\n

Your personal data is collected by our Clinic through different channels while performing our activities as Data Controller in order to ensure full compliance with the legislation, our policies and the obligations undertaken. Your personal data may be processed and transferred for the purposes specified in this Clarification Text within the scope of the personal data processing conditions specified in Articles 5 and 6 of the Law and the stipulated regulations in accordance with the basic principles stipulated in the Law.<\/p>\n

Your personal data will be processed for specific, clear and legitimate purposes in accordance with the law and the rule of honesty, in connection with the purposes of processing, limited and measured, accurate and up-to-date.<\/p>\n

To fulfil all diagnosis and treatment services in the field of dermatology and medical aesthetics,
\nFulfilment of our legal obligations under the Basic Law No. 3359 on Health Services, the Decree Law No. 663 on the Organisation and Duties of the Ministry of Health and its Affiliated Institutions, the Regulation on the Processing of Personal Health Data and Ensuring Privacy and other relevant regulations,
\nProtection of public health, preventive medicine, medical diagnosis, treatment and care services,
\nCreation of patient follow-up forms,
\nKeeping protocol books,
\nMaintaining information about your health data that must be kept in accordance with the relevant legislation,
\nFollow-up of appointments,
\nRealisation of after-service services,
\nDetermining the strategies related to the field of activity of our practice,
\nInvoice issuance and follow-up of reconciliations,
\nProviding communication during the material procurement phase and post-purchase invoice controls,
\nConfirmation of your relationship with the contracted institutions,
\nSharing the requested information with private insurance companies within the scope of financing health services,
\nTaking all necessary technical and administrative measures within the scope of data security regarding the systems and applications in our practice,<\/p>\n

Analysing your use of health services and storing your health data in order to develop and improve the health services we provide to you,
\nConfirmation of identity,
\nManaging patient satisfaction and complaints and responding to incoming questions,
\nCarrying out studies to improve the quality of service and treatment and providing better service,
\nProviding patients with the benefits of specialised organisations in order to receive services and technology services in areas that are not in their field of expertise,
\nKeeping current accounts,
\nProcurement of services from external sources,
\nFinancial reconciliation with relevant business partners and other third parties regarding the products and services offered,
\nProviding the necessary information and approvals in line with the requests and audits of regulatory and supervisory bodies and official authorities, as well as from the authorities that will examine the applications,
\nMaintaining the information regarding the data that must be kept in accordance with the relevant legislation,
\nKeeping visitor records within the framework of ensuring building security,
\nIn terms of employees; the creation of a personal file, determining whether they are capable of fulfilling the requirements of the job on a permanent basis, private health insurance,<\/p>\n

In return for the work done by the employees during the month; salary calculation, SSI premiums, overtime-salary payments,
\nRegular and complete transmission of all kinds of labour receivables and compensation payments of the dismissed employee to the correct channels,
\nIt may be processed for the purpose of implementing training, organisation, travel and other transactions for employees.
\nIn addition, without being limited to these, Exp. Dr. Lect. Eylem Ar\u0131kan during and after the service, development, analysis of products and services, financial reporting and analysis, legal follow-up and similar purposes. Your processed personal data; In accordance with the basic principles stipulated by the Law and within the personal data processing conditions and purposes specified in Articles 8 and 9 of the Law, in order to carry out our activities in accordance with the legislation and our policies, in order for our business units to carry out the necessary work in this direction, it can be shared with our employees within our clinic, in line with the purpose of the service we provide or with regulatory supervisory institutions, our suppliers, our business partners at home and abroad, and legally authorized public institutions and organizations, and may be transferred domestically and abroad in cases stipulated by the relevant legislation.<\/p>\n

IV. Your Rights as a Data Subject under the Law<\/strong><\/p>\n

Pursuant to Article 11 of the Law, you have the following rights regarding your personal data;<\/p>\n

To learn whether your personal data is being processed,
\nRequest information if your personal data has been processed,
\nTo learn the purpose of processing your personal data and whether they are used in accordance with their purpose,
\nTo know the third parties to whom personal data are transferred domestically or abroad,
\nTo request correction of personal data in case of incomplete or incorrect processing and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
\nTo object to the emergence of a result to the detriment of the person himself\/herself by analysing the processed data exclusively through automated systems,
\nAlthough it has been processed in accordance with the provisions of the Law and other relevant laws, to request the deletion or destruction of personal data in the event that the reasons requiring its processing disappear and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
\nIn case of damage due to unlawful processing of personal data, to demand the compensation of the damage.<\/p>\n

In the application containing your explanations regarding the right you have as a personal data owner and which you will make in order to exercise your rights mentioned above and which you request to exercise; the matter you request must be clear and understandable, the subject you request must be related to your person or if you are acting on behalf of someone else, you must be specifically authorised in this regard and this authorisation must be documented, the application must contain identity and address information and documents certifying your identity must be attached to the application.<\/p>\n

The applications made to us will be returned in writing or electronically within 30 days at the latest from the date of receipt of your request.<\/p>\n
